Please bring the following items when registering at Daphne East:

Two proofs of residence which must show physical street address.  One proof must include a purchase, mortgage, or lease/rental agreement to be verifed Plus a current utility bill.

The original state issued birth certificate

The Social Security Card ( not a copy)

The Alabama Immunization Record (Blue Card)
